29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection:    The employer did not have an assured equipment grounding program or a ground fault circuit interrupter for 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building.  A red two wire extension cord was used to charge batteries on the roof exposing employees to electrical hazards.

29 CFR 1926.405(a)(2)(ii)(J): Extension cord sets used with portable electric tools and appliances were not of the three-wire type:       The company used a two wire red extension cord to provide service to a battery charging station which was located on the roof of the building, and employees were exposed to possible electrical burns due to a lack of a ground cord.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:      The employer did not have each employee engaged in roofing activities on an approximately 120 foot long by 49 foot wide and 12 foot high low slop roof protected by the use of monitoring system, a warning line system, a guardrail system, a safety net system,  a personal fall arrest system, or any combinations of systems.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.      The employer did not have a Louisville Extension Ladder, Model #L-2222-24 with damaged leg supports marked or tagged with "Do Not Use", and withdrawn from service until it was repaired.   The employee used the ladder to access/egress to and from a 12 foot high roof.
