29 CFR 1910.269(d)(8)(iv): Whenever outside servicing personnel are to be engaged in activities covered by paragraph (d) of this section, the on-site employer and the outside employer shall inform each other of their respective lockout or tagout procedures, and each employer shall ensure that his or her personnel understand and comply with restrictions and prohibitions of the energy control procedures being used:      On March 14, 2015, the employer failed to ensure that its employees understood and complied with the procedures, restrictions, or prohibitions or the energy control procedures being used.

29 CFR 1910.269(d)(8)(v)(B): The system operator did not place and remove tagout devices in place of the authorized employee under paragraphs (d)(4), (d)(6)(iv), and (d)(7)(iv) of this section:     On March 14, 2015, the employer directed its contractor to remove three electrical insulators in close proximity to energized parts, located in Cubical 9, Bus 12, but failed to ensure that their system operator de-energized the equipment being maintained before the start of work.  The employee was exposed to electrocution/high voltage electric shock.
