29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: 29 Longmeadow Lane, Scarborough - On 3/9/15 an employee fell from the garage roof approximately 9 feet to the asphalt driveway below. He was not protected by a guardrail system, safety net system, or personal fall arrest system when standing with his left foot on an extension ladder and his right foot on the garage roof while shoveling snow away from the roof penetration.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: 29 Longmeadow Lane, Scarborough - On 3/9/15 an employee fell from the garage roof approximately 9 feet to the asphalt driveway below. He was not instructed to use personal fall protection while on a roof nor was it company practice to use fall protection while on roofs.
