29 CFR 1926.451(b)(1):   Each platform on all working levels of scaffolds was not fully planked or decked between front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  Worksite located at the intersection of Cumberland Road and 205th Streets, Elkhorn, NE:  The working level of a scaffold was not fully planked which decreases the working area for the employees.  They were exposed to falls of over 12 feet.     Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

29 CFR 1926.451(c)(2):   Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:     Worksite located at the intersection of Cumberland Road and 205th Streets, Elkhorn, NE:  A fabricated frame scaffold located at the site did not have base plates installed at each lower frame location.  This increases the potential for shifting of the scaffold while in use and possible collapse.      Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. 29 CFR 1926.451(e)(1):   When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.   Worksite located at the intersection of Cumberland Road and 205th Streets, Elkhorn, NE:  The fabricated frame scaffold observed at the job site was not constructed with an acceptable scaffold access method that meets the requirements listed above.        Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

29 CFR 1926.451(f)(3):   Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  Worksite located at the intersection of Cumberland Road and 205th Streets, Elkhorn, NE:  The fabricated frame scaffold observed at the job site had multiple defects (lack of guardrails, incomplete decking, missing base plates, and a bent cross brace) that should have been detected through inspections conducted prior to each work shift.        Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

The employees were exposed to falls of greater than 12 feet while working.      Abatement certification and abatement documentation is required for this violation. 29 CFR 1926.454(a):   The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.   Worksite located at the intersection of Cumberland Road and 205th Streets, Elkhorn, NE as well as throughout the company:  Employees working a fabricated frame scaffold had not received training as required by the standard.  This training should include but is not limited to the hazards associated with the type of scaffold being used.      Abatement certification and abatement documentation is required for this violation.
