On or about March 21, 2015, the employer failed to inform OSHA within 8 hours after the death of one of their employees from a work related accident. The employer was advised of the victims death at approximately 5:15 PM CST on March 21, 2015, and OSHA was notified by the Human Resource Manager at approximately 9:25 AM EST on March 23, 2015.
