29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a.  On or about March 21, 2015, the employer did not have written procedures to control the unexpected start-up of vehicles during servicing and maintenance activities, exposing employees to crushing and struck-by hazards.

29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident:  a.  On or about March 21, 2015, the employer failed to inform OSHA within 8 hours after the death of one of their employees from a work related accident.  The employer was advised of the victims death at approximately 5:15 PM CST on March 21, 2015, and OSHA was notified by the Human Resource Manager at approximately 9:25 AM EST on March 23, 2015.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  a.  On or about March 23, 2015, employees used hazardous chemicals such as, but not limited to acetone, wood fillers, glues, greases, and lubricants while working at the facility and the employer did not provide an explanation of the new labeling system and safety data sheet.
