29 CFR 1910.269(o)(2)(i): The employer did not establish and enforce work practices for the protection of each employee from the hazards of high-voltage or high-power testing at all test areas, temporary and permanent that include, at a minimum, test area safeguarding, grounding, the safe use of measuring and control circuits, and a means providing for periodic safety checks of field test areas: (a) On or about March 11, 2015 and at times prior to that date, for the employees exposed to electrical shock hazards while operating a VON model XF 16-1280, Serial Number 610262, Arc-Reflection test system to locate an underground electrical fault at the vicinity of 182nd Street & 401st Avenue, Frankfort, South Dakota.
