29 CFR 1904.39(a)(2): 29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. On March 09, 2015, at the worksite located at One Scroll Drive, Arkadelphia, Arkansas 71923, an employee sustained an amputation. The employer failed to report to OSHA the work-related amputation within twenty-four hours as required.
