29 CFR 1910.212(a)(3)(ii):Point of operation guard were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:      1208 Progress Blvd. Elberton, GA (Batching Dept.) - Employees were exposed to struck-by hazard.  Employees are operating the patty former machines without having the metal cover guard properly secured to prevent employees from coming in contact to the moving parts (point of operation) to form hamburger patties. The patty former machines was not adequately guarded at the point of operation. Employees can lift up the metal cover (guard) while machine is operating.  Also, there was interlock switch and/or light curtain to stop the machine from operating once the metal cover is moved/lifted, on or about 03/23/15.    a. The company's written lockout/tagout specific procedures were found with the following deficiency on each patty former machine written specific lockout/tagout procedures.   The energy control procedures did not specifically outline the steps for the removal of locks and restoring the lockout/tagout equipment and restoring the lockout/tagout equipment back into services, on or about 03/23/15.  a.
