29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented: a) 21 Bernart Court, Hillsborough, NJ: Employees were observed performing roofing operations at a height of approximately 24' to the ground below with no fall protection in place, exposing them to fall hazards. Condition observed on or about March 24, 2015. LDO Exteriors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 981814 Citation number 1, item number 1 and was affirmed as a final order on August 23, 2014, with respect to a workplace located at 787 Carpenter Road, North Brunswick, NJ.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired: a) 21 Bernart Court, Hillsborough, NJ: Employees were observed utilizing a 28' unmarked aluminum portable ladder to access the roof that had bent rungs, which could effect the structural stability of the ladder, exposing them to fall hazards. Condition observed on or about March 24, 2015. LDO Exteriors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(16), which was contained in OSHA inspection number 981814, citation number 1, item number 1 and was affirmed as a final order on August 23, 2014, with respect to a workplace located at 787 Carpenter Road, North Brunswick, NJ.
