On or about March 4, 2015, and times prior thereto, storage area for scrap sheet metal in docks 11 and 12 was not kept clean and orderly.  Sheet metal hangs over edges of bins.  Employees were exposed to the hazard of being struck by falling sheet metal and/or cut by sheet metal hanging out of bins.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  On or about March 6, 2014, and times prior thereto, employees are not provided with fall protection after leaving access stand on B-52s.  Employees walk four to five feet on top of wing with no fall protection to reach self retracting lifelines.  Employees are exposed to the hazard of falls.    The U.S. Department of Defense, Tinker Air Force Base, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.132(a), which was contained in OSHA inspection number 314930058, citation number 1, item number 4, and was affirmed as a final order on June 24, 2011.
