29 CFR 1910.213(h)(4): Installation shall be in such a manner that the front end of the unit will be slightly higher than the rear, so as to cause the cutting head to return gently to the starting position when released by the operator.  a) On or about Wednesday, March 25, 2015 in the Small Motor Shop, the table saw did not return gently to the starting position when released.  Employees were exposed to contact with a moving blade which could lead to cuts, scrapes, and possible amputations.

29 CFR 1910.215(a)(4): Work rests. On offhand grinding machines, work rests shall be used to support the work. They shall be of rigid construction and designed to be adjustable to compensate for wheel wear. Work rests shall be kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. The work rest shall be securely clamped after each adjustment. The adjustment shall not be made with the wheel in motion.  a) On or about Tuesday, March 24, 2015 in the Utilities Carpenter Shop, the grinding wheel work rest was not properly adjusted to a maximum opening of one-eighth inch.  Employees were exposed to possible struck by hazards which could result in punctures, loss of body parts or possible amputations.  b) On or about Tuesday, March 24, 2015 in fire station 1 work bay, the grinding wheel work rest was not properly adjusted to a maximum opening of one-eighth inch. Employees were exposed to possible struck by hazards which could result in punctures, loss of body parts or possible amputations.

Employees were exposed to live electrical parts which could lead to electrical burns and possible electric shock.

In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  a) On or about Wednesday, March 25, 2015 in the sprinkler riser room of the Paul H. Douglas building, electrical junction boxes were missing covers.  Employees were exposed to electrical hazards which could result in electrical burns and possibly shock.

29 CFR 1910.305(j)(2)(i): All 15- and 20-ampere attachment plugs and connectors shall be constructed so that there are no exposed current-carrying parts except the prongs, blades, or pins. The cover for wire terminations shall be a part that is essential for the operation of an attachment plug or connector (dead-front construction). Attachment plugs shall be installed so that their prongs, blades, or pins are not energized unless inserted into an energized receptacle. No receptacles may be installed so as to require an energized attachment plug as its source of supply.  a) On or about Wednesday, March 25, 2015 in the small motor shop building, there were numerous broken electrical receptacle plates/covers throughout the work area. Employees were exposed to energized, electrical wiring which could result in burns, shock or electrocution.
