The area beneath the OSB and/or shingles was not provided with additional support.  Condition observed on or about 03/24/15. Condition observed on or about 03/24/15.  b- Phase 1, South East Side, 559 Route 130 North, Hamilton NJ: Two employees, working from a pump jack scaffold whose legs were set on the ground below, were exposed to fall hazards in excess of 15 feet 5 inches to the ground surface below, when the pump jack scaffold was missing end stops along the scaffold's East side. This employee had not received scaffold hazard identification training, from the company, in order to control or minimize the hazards.  Condition observed on or about 03/24/15.  b- Phase 1, South East Side, 559 Route 130 North, Hamilton NJ: One employee, working from a pump jack scaffold whose legs were set on 5/8" OSB (covered with shingles) on an eave of a pitched roof, was exposed to fall hazards in excess of 25 feet to the ground surface below, when the pump jack scaffold was missing end stops, was missing mid and top rails along the scaffold's West side, and was set on a pitched and unstable footing.  This employee had not received scaffold hazard identification training, from the company, in order to control or minimize the hazards.  Condition observed on or about 03/24/15.  c- Phase 1, South East Side, 559 Route 130 North, Hamilton NJ: Two employees, working from a pump jack scaffold whose legs were set on the ground below, were exposed to fall hazards in excess of 15 feet 5 inches to the ground surface below, when the pump jack scaffold was missing end stops along the scaffold's East side.  These employees had not received scaffold hazard identification training, from the company, in order to control or minimize the hazards.  Condition observed on or about 03/24/15.
