(a) In aisle 6-50 along the Northwest, rear wall of the warehouse at 60 Webster Place Worcester, MA: On or about March 25, 2015, the employer did not ensure the exit was unobstructed due to placement of a pallet of merchandise and two stacked pallets with loose fiberboard on top which blocked the exit.    (b) In Aisle 85/86, near the employee break area, of the warehouse at 60 Webster Place Worcester, MA:  On or about March 25, 2015, the employer did not ensure the exit was kept unobstructed due to placement of a pallet containing a fiberboard box, pieces of plywood and two stacked chairs which blocked the exit.

Bags, containers, bundles, etc., stored in tiers were not stacked, blocked, interlocked and limited in height so that they were stable and secure against sliding or collapse.  (a) In warehouse room 6 at 60 Webster Place Worcester, MA: On or about March 24, 2015, the employer did not ensure that two, double-stacked pallets of ninety-one (91), fifteen (15) pound boxes of wood market umbrellas were stable and secured against collapse in that some lower boxes were crushed causing the two stacks to be leaning exposing employees to struck and crushed-by hazards.  (b) In warehouse room 5 at 60 Webster Place Worcester, MA: On or about March 24, 2015, the employer did not ensure that three, stacked pallets of forty-five (45), nineteen (19) pound boxes were stacked and blocked so they were stable and secure in that the lower pallet contained partially crushed boxes causing the stacked tower of merchandise to lean exposing employees to struck and crush-by hazards.

In the battery charging area in the warehouse at 60 Webster Place Worcester, MA: On or about March 24, 2015 the employer did not provide sufficient access and working space about eight emergency disconnects for powered industrial truck (PIT) battery chargers to permit ready and safe operation and maintenance of the equipment in that the PIT battery chargers were located approximately 20 inches from the wall where the disconnects were located.
