29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) Synergy PSM Corporation; On or about March 21, 2015, the employer failed to ensure that energy control procedures were utilized to control potentially hazardous energy sources, electrical and pneumatic, while employees performed cleaning operations inside the clean room paint booth of Continental Structural Plastics (CSP). Employees performed cleaning activities were exposed to struck by and caught between hazards in the event that ABB AS robots located inside the paint booth were to become energized.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):    a) Synergy PSM Corporation; On or about March 21, 2015, the employer failed to ensure that employees isolated the electrical and pneumatic energy of the ABB AS Robotic cells located inside the clean room paint booth on Continental Structural Plastics (CSP) prior to engaging in cleaning operations.  Employees performing cleaning activities were exposed to struck by and caught between hazards in the event that paint robots located inside the paint booth become energized.

Employees performing cleaning activities were exposed to struck by and caught between hazards in the event that paint robots located inside the paint booth become energized.

Employees were exposed to struck by and caught between hazards in the event that ABB AS robots located inside the paint booth become energized.

29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form:  a) Synergy PSM Corporation; On or about April 25, 2015 the employer failed to evaluate the respiratory hazards for those employees who were exposed to hazards associated with paint vapors such as, but not limited to, headaches, drowsiness, nausea, and asthmatic conditions from exposure to chemicals such as xylene, n-butyl acetate and hexamethylene diisocyanate homopolymer (HDI).
