Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  On or about March 11, 2015 at Tyson Foods located at 3800 Progress, North Little Rock, AR 72114, an employee of Kirby Specialties Corporation sustained an injury to his right leg and ankle when he lost his footing and fell from a cage ladder. The hospitalization was reported on March 13, 2015, which was not within the stipulated 24 hour timeframe.
