29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Continental Structural Plastics; On or about March 21, 2015, the employer failed to ensure that energy control procedures were utilized to control potentially hazardous energy sources, electrical and pneumatic, of ABB AS robots when employees who entered the clean room paint booth to change out the fluid delivery lines. Employees were exposed to struck by and caught between hazards in the event that ABB AS robots located inside the paint booth were to become energized. b) Continental Structural Plastics; On or about June 25, 2015, the employer failed to ensure that energy control procedures were utilized to control potentially hazardous energy sources when employees enter the clean room paint booth to replace broken paint guns on a ABB AS robot. Employees were exposed to struck by and caught between hazards in the event that ABB AS robots located inside the paint booth became energized.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) Continental Structural Plastics; On or about March 21, 2015, the employer failed to ensure that employees who entered the clean room paint booth to change out the fluid delivery lines isolated hazardous energy sources, pneumatic and electric, of the ABB AS robots. Employees were exposed to struck by and caught between hazards in the event that ABB AS robots located inside the paint booth where to become energized. b) Continental Structural Plastics; On or about June 25, 2015, the employer failed to ensure that employees who entered the clean room paint booth isolated hazardous energy sources, pneumatic and electric, of the ABB AS robots when a broken paint gun of a robot needed replaced. Employees were exposed to struck by and caught between hazards in the event that ABB AS robots located inside the paint booth where to become energized.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: a) Continental Structural Plastics; On or about June 25, 2015, the employer did not ensure that employees who entered the clean room paint booth to perform maintenance tasks were adequately trained in the skills required for the safe application, usage, and removal of energy controls. Employees were not trained to isolate all energy sources to protect against the unexpected energization of the ABB AS Robots located inside the paint booth.
