29 CFR 1926.502(b)(1): The top edge height of the top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level: a) White Springs at Providence, Lot 16, Worksite - on or about March 24, 2015, subcontractor employees were performing frame carpentry work where the guardrails installed in the window frames by the fabricator were approximately 35.5 to 36.5 inches above the walking/working level.

29 CFR 1926.1402(c)(1): The employer did not ensure that ground preparations necessary to meet the requirements in paragraph (b) were provided: a) White Springs at Providence, Lot 17, Worksite - on or about March 24, 2015, a Manitex SX 102 Truck Mounted Crane was in use to lift trusses to the necessary assembly height while set upon ground conditions which were not maintained in a condition of being firm, drained, and graded.
