29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  On or about March 24, 2015 the employer did not ensure that scissor lift(s) were inspected and removed when not in safe operable condition. Scissor lifts were missing railing and/or gates, railing on lifts were held with baling wire, operator control panels were not legible, the electrical components and wiring were spliced and/or exposed. These conditions exposed employees to fall hazards and hazards associated with electrical shock.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:    a) On or about March 24, 2015 the employer did not ensure that an extension cord, powering a miter saw, was equipped with a ground prong.    b) On or about March 24, 2015 the employer did not ensure that a miter saw's cord was equipped with a ground prong.    c) On or about March 24, 2015 the employer did not ensure that a JLG scissor lift's cord was equipped with a ground prong.    This condition exposed employees to hazards associated with electrical shock.

29 CFR 1926.405(g)(2)(iii): Flexible cords were not used in continuous lengths without splice or tap:  a) On or about March 24, 2015 the employer did not ensure that a flexible cord was used in continuous length without a splice. The spliced cord was connected to an air compressor on a Genie lift.  b) On or about March 24, 2015 the employer did not ensure that a flexible cord attached to a miter compound saw was used in continuous length without a splice.  This condition exposed employees to hazards associated with electrical shock.
