Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause serious physical harm to employees in that employees were exposed to a point of operation hazard:    a)  Fabrication Area, on or about 3/25/15:   Employer had not developed and documented a safe distance safeguarding program, including periodic inspections, for employees using the Cincinnati 230 ton press brake and the LVD 50 ton press brake.      Abatement certification must be submitted for this item.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions:    a)  Fabrication Area, on or about 3/25/15:  Flexible conduit was pulled loose from the fitting on a Mueser radial drill, exposing conductors to damage.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a)  Fabrication Area, on or about 3/25/15:  Hold downs and the point of operation on a Pexto shear were not adequately guarded in that there was an opening in the guard on the left side measuring 4" X 5".    b)  Fabrication Area, on or about 3/25/15:  Rotating chuck and spindle on a Mueser radial drill were not guarded.      The Industrial Fabricating Inc. was previously cited for a violation of this Occupational Safety and Health standard which was contained in OSHA Inspection number 56398, Citation number 1, Item number 5 and was affirmed as a final order on 10/17/12, with respect to a workplace located at 4 Collamer Circle, East Syracuse, New York 13057.    Abatement documentation must be submitted for these items.
