29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment:  A personal protective equipment hazard assessment was not performed to determine the type of personal protective equipment needed such as chemical goggles, face shields, gloves, and etc., for employees adding water to batteries on fork trucks, using Sherwin Williams paints, and etc.   Employees working in the warehouse were exposed to health hazards.

29 CFR 1910.178(l)(4)(i): Refresher training, including an evaluation of the effectiveness of that training, was not conducted at least once every three years as required by paragraph (l)(4)(ii) to ensure that the operator had the knowledge and skills needed to operate the powered industrial truck safely:   At the establishment the employer did not perform powered industrial operator refresher training for employees driving the Nissan forklift and the Atlet order picker that were being used to move materials throughout the warehouse.   Employees were exposed to fall and struck by hazards.

29 CFR 1910.1200(h)(1): Employers did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees had not previously been trained about was introduced into their work area and chemical-specific information was not always available through labels and safety data sheets:  At the establishment employees using hazardous chemical products including but not limited to the following sulfuric acid, Sherman Williams paints, and etc., were exposed to health hazards due to no effective training to recognize and avoid health hazards associated with hazardous chemical products.
