29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:   a. At the facility located on 159 Progress Circle, Venice, FL, employees wearing, 3M, N-95, dust/mist respirators, while engaged in activities related to the manufacturing of cabinets and granite. Employees are exposed to the health hazards associated with chemicals such as, but not limited to, silica and wood dust. No training and/or information had been provided, on or about, 03/25/2015.
