29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Sanding/Machine Room; The employer failed to ensure that a Dayton Drill press Model 3Z919B Lot#106472 used by the employees was equipped with a chuck guard.  Employees were exposed to laceration hazards.

Employees were exposed to laceration hazards.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    a) MACHINE ROOM: The employer had not established or implemented a written Respiratory Protection Program.  Employees were required to wear disposable N95 respirators while performing grinding and sanding activities while making prosthetics limbs.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:    a) Oven Room; Employees were required to work with chemicals such as, but not limited to rubber cement, epoxy vinyl ester resin, and acetone (flammables) while manufacturing prosthetic limbs.  The employer did not provide training on the new label elements and safety data sheets format.
