29 CFR 1926.1053(b)(6): Ladders shall be used only on stable and level surfaces unless secured to prevent accidental displacement.  WORKSITE: The Louisville model L2223-24, Type II 24-foot aluminum extension ladder, which was placed on frozen and slippery ground and used by an employee while installing vapor barrier on the garage wall, was not properly set-up and secured to prevent from displacement.    On or about 3/18/2015, the above ladder was displaced while the employee was performing the above task, causing the employee fell to the ground. The employee sustained facial and wrist injuries from the incident.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders shall be used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support).  WORKSITE: The Louisville model L2223-24 Type II24-foot aluminum extension ladder, which was used by an employee while installing vapor barrier was setup such that the base of the ladder exceeded more than one-quarter of the working length of the ladder.
