29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA: (a) 10 Northeast Drive, Hershey Pennsylvania - An employee suffered a hospitalization on March 4, 2015 as a result of a broken ankle from a slip on ice. The employer failed to report this work-related amputation to OSHA within 24 hours. (The employer reported this hospitalization to OSHA on March 18, 2015.) No abatement certification or documentation required.
