On the roof of a residential property, employees were connected to vertical lifelines that were tied in knots at the anchorage point exposing the employees to a 24 foot fall hazard, on or about 3/25/2015.

On the 5/12 pitched roof, fall protection was not used during the installing of plywood sheeting, exposing the employees to a 24 foot fall hazard, on or about 3/25/2015. Costa Homes, Inc., dba P&C Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 920722, citation number 1, item number 1 and was affirmed as a final order on 8/9/2013, with respect to a workplace located at 1310 Riserva Ln, Palm Harbor, FL 34685.
