29 CFR 1910.217(c)(1)(i): The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press(s): a) On or about 3/24/2015, Shop Area: A Niagara mechanical power press, #80, had guards on its infeed and outfeed sides that were not adjusted to prevent access to the point of operation. b) On or about 3/24/2015, Shop Area: A Stantec G1-80 mechanical power press, #59, had openings in its guards that would allow access to the point of operation. c) On or about 3/24/2015, Shop Area: A V & O mechanical power press, was missing a guard on its corner and a side guard was wired in an open position, which allowed access to the point of operation. d) On or about 3/24/2015, Shop Area: A Minster mechanical power press, #16, had an opened guard which allowed access to the point of operation. e) On or about 3/24/2015, Shop Area: A Minster mechanical power press, #44, had a guard with openings and a lack of guard in areas which allowed for access to the point of operation. f) On or about 3/24/2015, Shop Area: A Bliss mechanical power press, #65, had openings in the guards, and had a guard that was wired in the open position, which allowed for access to the point of operation.
