29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as severe cuts or lacerations. (a) Production room: Employees using large sharp knifes to cut pizza dough were not provided with appropriate hand protection and as such were exposed to hazards such as severe cuts or lacerations. On or about 03/26/2015 an employee suffered a serious cut to his thumb performing this work. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.1030(g)(2): The employer did not train each employee with occupational exposure in accordance with the requirements of this section. a) Production room; On or about 3/25/2015, an employee weighing and cutting dough received a severe cut from a knife. As such, employees working in this area and the manager that cleaned the area are exposed to potential occupational exposure to human blood. The employer did not train each employee in the proper protection procedures related to blood borne pathogens. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
