29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   (a). Worksite located at 3959 Broadway, Kansas City, Missouri- Employee was observed working and walking on the edge of the parapet wall and was not protected from falling by the use of any means of fall protection.

Worksite located at 3959 Broadway, Kansas City, Missouri- Employee exposed to fall hazards was not trained on the recognition and avoidance of fall hazards and methods needed to prevent or protect him from falling.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   (a). Worksite located at 3959 Broadway, Kansas City, Missouri- Employee was exposed to a fall hazard in that the ladder the employee was using to access the rooftop did not even reach the edge of the parapet wall.
