(a) On or about 03/25/2015 - Cabinet shop, combustible dust generated from the woodworking operations, such as but not limited to, cutting, sawing, planning, sanding and milling was allowed had accumulated on the equipment, electrical equipment, electrical motors, walls, pipes, and beams from 1/8 inch to 2 inches in depth, exposing employees to a fire hazard. The dust was also allowed to accumulate inside equipment.
