29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition:  a.  Dept. 35, Optimizer - On or about March 25, 2015, a foot petal and the flexible cord to the foot petal was stretched across the landing of a stairway exposing employees to a tripping hazard.    b. Debarker Loading Dock and Debarker Control Room - On or about March 25, 2015, a water hose was on the floor close to the edge of the dock and an air hose was on the floor next the Miller welder thereby exposing employees to a tripping hazard.

Carbide Shop - On or about March 25, 2015, the Baldor bench grinder tongue guard was installed incorrectly and would not have prevented the operator from being struck by pieces of the wheel if the wheel disintegrates.  The tongue guard was not above the grinding wheel.
