Dept. 35, Optimizer - On or about March 25, 2015, a foot petal and the flexible cord to the foot petal was stretched across the landing of a stairway exposing employees to a tripping hazard. b. Debarker Loading Dock and Debarker Control Room - On or about March 25, 2015, a water hose was on the floor close to the edge of the dock and an air hose was on the floor next the Miller welder thereby exposing employees to a tripping hazard.
