29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a) On the south side of the home, on the first story, where an employee was not wearing a hard hat while working directly below two other employees who were installing rafters on the second story, exposing him to head injuries from falling lumber.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) In the south side of the home, on the first story, where  one (1) employee was installing rafters and was not protected from falling 11 feet to the ground below.    b) In the south side of the home, on the second story, where two (2) employees were installing rafters and were not protected from falling 18 feet to the ground below.
