29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):    a) On or about in the shop area located at 51 North Gates Avenue; employer failed to protect employees from caught between hazards when they allowed placing the press bake in jog mode during the servicing and maintenance activities, such as but not limited to, die changes and set up.       b) On or about 03/26/15 in the shop area located at 51 North Gates Avenue; employer failed to protect employees from struck by hazards of the moving grinding wheel, when they allowed using the stop button as the sole means of energy control during service and maintenance activities, such as but not limited to, grinding wheel removal. 29 CFR 1910.217(c)(2)(i)(d): Point of operation guard(s) on mechanical power press(es) did not utilized fasteners not readily removable by the operator, so as to create minimize the possibility of misuse or removal of essential parts:     a) On or about 03/16/17 at the Bliss power press located at 51 North Gates Avenue; employer exposed employees who operated the power press to caught between hazards when the ram and the point of operation guards were secured with thumb screws which were easily removable.      ABATEMENT CERTIFICATION REQUIRED
