29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    a) Division 7, Inc. at 1627 Ski Hill Rd., Breckenridge, CO 80424:  On or before March 25th, 2015 employees spraying and assisting with spray foam insulation activities were exposed to health hazards associated with MDI and toluene - 2, 6 diisocyanate while using a 2-part foam material and the employer failed to ensure that employees had received appropriate hazard communication training which included discussion on dermal exposures and sensitizers.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  a) Division 7, Inc. at 1627 Ski Hill Rd., Breckenridge, CO 80424:  On or before March 25th, 2015 an employee assisting with spray foam insulation activities was required to wear a 3M half mask outfitted with combination P100/Organic Vapor cartridges and the employer did not ensure that the employee had received a medical evaluation prior to being required to wear the respirator in the workplace.  This condition exposed the employee to potential cardiopulmonary hazards stemming from an increased respiratory burden.

29 CFR 1910.134(g)(1)(i):  The employer shall not allow an employee who has facial hair which interferes with the sealing surface of the respirator to wear a tight-fitting respirator in the workplace:  a) Division 7, Inc. at 1627 Ski Hill Rd., Breckenridge, CO 80424:  On or before March 25th, 2015 an employee assisting with spray foam insulation activities was required to wear a 3M half mask outfitted with combination P100/Organic Vapor cartridges.  The employee had a substantial amount of facial hair which interfered with the sealing surface of the respirator and which could have exposed the employee to hazards if the respirator was used within a hazardous atmosphere.

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or  were not packed or stored to prevent deformation of the facepiece and exhalation valve:   a) Division 7, Inc. at 1627 Ski Hill Rd., Breckenridge, CO 80424:  On or before March 25th, 2015 an employee assisting with spray foam insulation activities was required to wear a 3M half mask outfitted with combination P100/Organic Vapor cartridges and the employer did not ensure that the employee's respirator was stored appropriately as to avoid environmental contamination.  This exposed the wearer to potential hazards in the event that the respirator was contaminated as it was found unprotected in the back of the job trailer where barrels of spray foam insulation components were kept.
