29 CFR 1926.300(b)(2): Moving part(s) of equipment exposed to contact by employees or which otherwise created a hazard, were not guarded:  (a) Front side, new townhome construction located at 415 Rose Garden Lane, Alpharetta, GA 30009: Employee was exposed to amputation hazards while working in close proximity to the recoil starter on the engine of the MED (Model 650) Trailer Mixer which was missing the cover exposing the operator to moving parts.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports:   a) East gable side, new townhome construction located at 415 Rose Garden Lane, Alpharetta, GA 30009: Employee was exposed to falls of approximately 12-feet, 7-inches to the lower level while working from a tubular welded scaffold platform which was not fully planked.   b) East gable side, new townhome construction located at 415 Rose Garden Lane, Alpharetta, GA 30009: Employee was exposed to falls of approximately 18-feet, 4-inches to the lower level while working from a tubular welded scaffold platform which was not fully planked.   c) East gable side, new townhome construction located at 415 Rose Garden Lane, Alpharetta, GA 30009: Employee was exposed to falls of approximately 23-feet, 3-inches to the lower level while working from a tubular welded scaffold platform which was not fully planked.  d) East gable side, new townhome construction located at 415 Rose Garden Lane, Alpharetta, GA 30009: Employee was exposed to falls of approximately 29-feet, 9-inches to the lower level while working from a tubular welded scaffold platform which was not fully planked.

29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph.  a) East gable side, new townhome construction located at 415 Rose Garden Lane, Alpharetta, GA 30009: Employees were exposed to collapse/falls of up to 29-feet, 9-inches while working from a tubular welded scaffold system which was 6 frame sections high without being tied into the structure to prevent tipping. The scaffold system was loaded at the top with bricks and the system was not tied into the building.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  a) East gable side, new townhome construction located at 415 Rose Garden Lane, Alpharetta, GA 30009: Employees were exposed to falls of up to 32-feet, 5-inches while working on tubular scaffold platforms which were not provided with a ladder for safe access. Employees climbed cross-bracing and masonry walk-thru style scaffold frames to access the working platforms when a ladder or other safe means of access was not provided.

(a) East gable side, new townhome construction located at 415 Rose Garden Lane, Alpharetta, GA 30009: An employee was exposed to fall hazards of approximately 12-feet, 7-inches to the ground level while engaged in masonry activities on a tubular welded scaffold platform in which guardrails were not provided for fall protection.  (b)  East gable side, new townhome construction located at 415 Rose Garden Lane, Alpharetta, GA 30009: An employee was exposed to fall hazards of approximately 18-feet, 4-inches to the ground level while engaged in masonry activities on a tubular welded scaffold platform in which guardrails were not provided for fall protection.  (c)  East gable side, new townhome construction located at 415 Rose Garden Lane, Alpharetta, GA 30009: An employee was exposed to fall hazards of approximately 23-feet, 3-inches to the ground level while engaged in masonry activities on a tubular welded scaffold platform in which guardrails were not provided for fall protection.
