29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, shall be kept cleared from work areas, passageways, and stairs, in and around buildings or other structures.   This violation occurred on March 26, 2015at a jobsite located at 1118 Misty Meadow Dr. Forney, TX 75126; on the north and south sides of the construction site, where a pile of debris had accumulated near the entrance to the jobsite, exposing employees to the hazards of lacerations and/or puncture injuries caused by stepping on nails.

This violation most recently occurred on March 26, 2015 , at a jobsite located at 1118 Misty Meadow Dr Forney, TX 75126; where at least two (2) employees used pneumatic nail guns without the use of protective eyewear.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:   This violation occurred on March 26, 2015 at a jobsite located at: 1118 Misty Meadow Drive Forney, TX 75126 where employees connecting a portable extension cord with the protective sleeve pulled out of the housing were exposed to an electrical hazard.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   This violation most recently occurred on March 26, 2015, at a jobsite located at 1118 Misty Meadow Dr. Forney, TX 75216 where at least two (2) employees working from a roof when installing roofing materials were not protected from falling and were exposed to a fall hazard greater than 10-feet to the ground below.
