29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items A-D of this section:                a) On or about March 27, 2015, at the above located workplace, employees were required to perform equipment inspection and other service and maintenance work. The employer's written lockout energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy  and provide employees the guidance necessary to effectively and safely control hazardous energy when they inspected, serviced or maintained the equipment:         i) The Lockout/Tagout Program (EHS-10D) required employees performing preventive maintenance to have the equipment in a zero energy state and locked out. The Empty Bottle Conveyor Gallon Line energy control procedures (LOTO-OL-CNV-MM-BOT) allowed employees performing preventative maintenance to use the E-Stop or Safety Trip Line.       ii) The Lockout/Tagout Program (EHS-10D) required employees performing preventive maintenance to have the equipment in a zero energy state and locked out. The Empty Bottle Sorter Swing Line energy control procedures (LOTO-OL-UNS-L02) allowed employees performing preventative maintenance to use the E-Stop.       iii) The Lockout/Tagout Program (EHS-10D) required employees performing preventive maintenance to have the equipment in a zero energy state and locked out. The 64 Position Net Weight Filler Swing Line energy control procedures (LOTO-OL-FLC-L02) allowed employees performing preventative maintenance and De-Jamming (entering the machine) to use the E-Stop.       v) The Lockout/Tagout Program (EHS-10D) training required employees to perform lockout procedures when clearing a jam or cleaning equipment. The Empty Bottle Conveyor Gallon Line energy control procedures (LOTO-OL-CNV-L04-MM-BOT), the Empty Bottle Sorter Swing Line energy control procedures (LOTO-OL-UNS-L02), the 64 Position Net Weight Filler Swing Line energy control procedures (LOTO-OL-FLC-L02) and the Empty Bottle Conveyor Gallon Line energy control procedures (LOTO-OL-CNV-L04-MM-BOT) allowed clearing a jam or cleaning equipment while using the E-stop and/or Safety Trip Line.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done:       a) On or about March 24, 2015, at the above located workplace, employees were required to perform equipment inspection (a servicing and/or maintenance operation). All energy isolating devices needed to control the energy to the machine were not operated in such a manner as to isolate the machine or equipment from the energy source(s). The employee was thereby exposed to the hazards associated with moving parts.

29 CFR 1910.219(f)(3): All sprocket wheels and chains that were less than seven (7) feet above the floor or platform were not enclosed:     a) On or about March 27, 2015, employees in the empty bottle truck unloading area were required to work adjacent to unguarded chains and sprockets on the empty bottle conveyors. The employees were thereby exposed to the hazards associated with the unguarded moving parts.
