29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a) Worksite - On or about March 27, 2015, employees are performing hazardous energy control on equipment and the employer did not develop a written lockout / tagout program, specific procedures and employee training to prevent injury from unexpected energizing, startup or release of stored energy.

29 CFR 1910.147(f)(2)(ii): The onsite employer did not ensure that his or her personnel understood and complied with restrictions and prohibitions of the outside employer's energy control procedures:       a) Worksite - On or about March 27, 2015, employees are working with equipment where contract employees are applying energy control devices, but the employer has not reviewed these procedures or conducted advisory training relating to restrictions and prohibitions of the outside employer's energy control procedures with their employees.
