29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.  On or about 8 March 2015, at the job site located at 228 N. 6th Street on the campus of Ouachita Baptist University in Arkadelphia AR 71923 the employer did not report an inpatient hospitalization until 10 March 2015.  An employee fell from a ladder resulting in inpatient hospitalization that was not reported until two days after the accident.
