29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold:     (a) At a new residential construction home, north side: On or about March 25, 2015, and at times prior thereto, an employee working from a pump jack scaffold was exposed to a fall hazard greater than 13 feet to the ground, while installing fiber cement siding on the exterior wall.     (b) At a new residential construction home, west side: On or about March 25, 2015, and at times prior thereto, an employee working from a pump jack scaffold was exposed to a fall hazard greater than 10.5 feet to the ground, while installing fiber cement siding on the exterior wall.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary:     At a new residential construction home, north and west side: On or about March 25, 2015, and at times prior thereto, employees working from two pump jack scaffolds that were not properly braced were exposed to a fall hazard, while installing fiber cement siding on the exterior wall.

29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:   At a new residential construction home, north and west side: On or about March 25, 2015, and at times prior thereto, employees working from a pump jack scaffold system were exposed to a fall hazard greater than 10.5 feet to the ground, while installing fiber cement siding on the exterior wall without being trained.
