The employee was not protected by a hard hat or protective helmet, on or about March 11th, 2015.

29 CFR 1926.451(b)(1):   Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports:   A) Exterior, On scaffold: Employees were exposed to fall hazards of approximately 14 and 21 feet while working on a fabricated framed scaffold that was not fully planked, on or about March 11th, 2015.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers, stairway-type ladders, ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    A) Exterior, On scaffold: Employees were exposed to fall hazards of up to approximately 21 feet while climbing the scaffold and cross bracing to get onto and off of the scaffold, on or about March 11th, 2015.
