29 CFR 1910.23(a)(5):   Every pit and trapdoor floor opening, infrequently used, shall be guarded by a floor opening cover of standard strength and construction. While the cover is not in place, the pit or trap opening shall be constantly attended by someone or shall be protected on all exposed sides by removable standard railings. Every pit and trapdoor floor opening, infrequently used was not guarded as required.  -    On or about 26 March 2015, in bay one of the main structure, the employer failed to ensure that a maintenance pit used by employees was guarded, exposing employees to fall hazards.

29 CFR 1910.177(d)(3)(i): The restraining barrier was not certified or was not capable of withstanding the maximum force that would be transferred to it during a rim wheel separation occurring at 150 percent of the maximum tire specification pressure for the type of rim wheel being serviced.      On or about 26 March 2015, outside, near bay two, the employer failed to ensure that a restraining barrier being used by employees to inflate tires, was certified by an approved certifying entity, exposing employees to struck by hazards.

On or about 26 March 2015 in the rear of the main structure, at ground level in the tire storage area, the employer failed to ensure that an active compressor in the employee work area had guards shielding its pulleys, exposing employees to caught between hazards.
