29 CFR 1926.405(b)(2): In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy: a) Room on southwest side of the North Commons - A cover was not provided on a permanently installed junction box which provided power to a temporary light and to a ground fault protected receptacle box mounted on a portable stand. b) Northeast wall of North Commons - A cover was not provided on a permanently installed receptacle box which provided power to a ground fault protected portable receptacle box. c) Northeast wall of North Commons - A cover was not provided on a permanently installed receptacle box which provided power to a ground fault protected portable receptacle box. d) Hall on south end of building - A cover was not provided on a permanently installed receptacle box which provided power to a ground fault protected receptacle box mounted on a portable stand.

29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: a) Room on southwest side of the North Commons - A non metallic sheathed cable providing power from a permanently installed junction box to a ground fault protected receptacle box mounted on a portable stand was not secured within 12 inches of the junction box and at least every 4 1/2 feet along its length. b) Room on southwest side of North Commons - A non metallic sheathed cable providing power from a permanently installed junction box to a temporary light was not secured within 12 inches of the junction box at least every 4 1/2 feet along its length. c) Northeast wall of North Commons - A non metallic sheathed cable providing power from a permanently installed receptacle box to a ground fault protected portable ground fault protected receptacle box was not secured within 12 inches of the receptacle boxes and at least every 4 1/2 feet along its length. d) Northeast wall of North Commons - A non metallic sheathed cable providing power from a permanently installed receptacle box to a ground fault protected portable ground fault protected receptacle box was not secured within 12 inches of the receptacle boxes and at least every 4 1/2 feet along its length. e) Hall on south end of building - A non metallic sheathed cable providing power from a permanently installed receptacle outlet to a ground fault protected receptacle box mounted on a portable stand was not secured within 12 inches of the junction box and at least every 4 1/2 feet along its length.
