29 CFR 1926.502(f)(1)(i): When mechanical equipment is not being used, the warning line shall be erected not less than 6 feet (1.8 m) from the roof edge. a. On or about March 27, 2015 - at the above addressed jobsite, all warning lines were erected closer than six (6) feet from the roof edge. Employees were thereby exposed to the hazards associated with falls of approximately twenty seven (27) feet to the ground.

29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted. a. On or about March 27, 2015 - at the above addressed jobsite, employees conducting flat roofing operations approximately twenty seven (27) feet above the ground were not protected from falling, thereby exposing the employees to fall hazards. THE CARE SHEET METAL & ROOFING, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.501(b)(10), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 531959, CITATION NUMBER 2, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON AUGUST 30, 2012 WITH RESPECT TO A WORKPLACE LOCATED AT 7137 W. 61ST STREET, CHICAGO, IL 60638.
