On or about March 27, 2015 - at the above addressed jobsite, employees conducting flat roofing operations approximately twenty seven (27) feet above the ground were not protected from falling, thereby exposing the employees to fall hazards. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.501(b)(10), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 531959, CITATION NUMBER 2, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON AUGUST 30, 2012 WITH RESPECT TO A WORKPLACE LOCATED AT 7137 W. 61ST STREET, CHICAGO, IL 60638.
