29 CFR 1926.102(a)(1): Eye and face protective equipment was not used when machines or operations presented potential eye or face injury: a) 75 Main Street, Woodbridge, NJ 07095. Employee was exposed to eye and face injuries when operating a Milwaukee 6 ¼ circular saw and a Hitachi, NV83A3 pneumatic coil nailer while installing roof sheathing while not wearing any eye and or face protection. Condition occurred on or about 03/27/15.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented: a) 75 Main Street, Woodbridge, NJ 07095. Employees were exposed to fall hazards while working on and from the roof of a three story building installing roof sheathing with no fall protection systems in place. Condition occurred on or about 03/27/15.
