29 CFR 1910.141(a)(5): Enclosed workplace(s) were not so constructed, equipped, and maintained to prevent the entrance or harborage of rodents, insects, or other vermin. A continuing and effective extermination program was not in place at the facility. Facility wide: On or about March 25, 2015, and at times prior thereto, mice activity including repeated sightings within the last two weeks were reported and has continued for over a year. Third and Fourth Floors: Dead mice were found in Module 4 and Module 5. In Module 6, live mice were sighted by employees.
