29 CFR 1926.501(a)(2): The employer did not determine if the walking/working surfaces on which its employees were to work had the strength and structural integrity to support employees safely:  a) 5237 Sparrow Lane, Worksite - on or about March 27, 2015, employees were observed climbing and walking on wood trusses which were still under suspended by the onsite crane which did not allow the strength and structural integrity to be definitively determined by the employer.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) 5237 Sparrow Lane, Worksite - on or about March 27, 2015, employees were observed working approximately 15 feet above a lower level were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) 5237 Sparrow Lane, Worksite - on or about March 27, 2015, employees were observed climbing an extension ladder used to access the top plates of a wood framed structure where the ladder side rails did not extend at least three feet above the upper landing surface.   Kistler Buildings, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1) which was contained in OSHA inspection number 314723206, citation number 2, item number 1b and was affirmed as a final order on April 28, 2011, with respect to a workplace located at 10635 Hamilton Blvd.,  Breinigsville, PA 18031.
