29 CFR 1926.100(a):   Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  Employees were exposed to struck by hazards due to the lack of head protection while working below other employees removing shingles and other debris from the roof.  The employer did not ensure that head protection was worn as necessary.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.102(a)(1):     Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    Employees were exposed to eye injury from the lack of protective safety glasses while using handheld pneumatic nail guns to affix shingles to the roof of a residential property.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.1053(b)(1):   Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   Employees used a portable metal extension ladder to access an 8/12 slope roof approximately 10 feet 8 inches above the ground. The ladder side rails did not extend at least 3 feet above the upper landing surface; they were extended approximately one foot above.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
