29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    a. On or about March 25, 2015, employees were permitted to work within 5-foot of the 13.2 KV energized power lines that were not protected against electrical shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means, an employee was electrocuted when the 20-foot iron channel rod he was removing came into contact with the energized power line.

29 CFR 1926.416(a)(3): Before work began, the employer did not ascertain by inquiry, direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring any person, tool, or machine into physical or electrical contact with the electric power circuit:    a. On or about March 25, 2015, the employer did not conduct a hazard assessment, which would have identified a 3-phase overhead power line system. The overhead power line system was not protected, in that the utilities company was not contacted so the lines could be de-energized or sleeved, which resulted in an electrocution.

29 CFR 1926.502(f)(1)(i): When no mechanical equipment was being used the warning line was not erected at least 6 feet (1.8m) from the roof edge:    a. On or about March 25, 2015, on a 22-foot commercial buildings roof, a warning line system was not established 6-foot back from the edge, exposing employees to a fall hazard.
