29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  1) Shop Area - The employer did not implement a lockout/tagout program to protect employees performing maintenance and changing blades on equipment such as, but not limited to lathe machine, cross cut saw and other equipment in the shop area.

29 CFR 1910.213(h)(1): The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the stock being cut to give maximum protection possible for the operation being performed:  1) Cross-Cut Area - Employees were using a cross-cut saw to cut wood where the blade was broken and not guarded. Employees were exposed to an amputation/cut hazard.

Employees were exposed to a struck-by hazard.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:  1) Shop, near cross-cut saw - The electrical outlet box was not completely enclosed with covers designed by the manufacturer.  The junction outlet box was cracked and damaged and did not fit properly.  The employer was using a plastic faceplate on metal junction box.  Employees were exposed to shock hazard from live parts.      2) Shop near Lathe machine - The electrical outlet box was not completely enclosed with covers designed by the manufacturer.  The electrical outlet box was covered with duct tape.  Employees were exposed to shock hazard from live parts.
