29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    On or about March 18, 2015, in the production area of the Yarnell's ice cream manufacturing facility, located at 320 S Spring St in Searcy, Arkansas, machine specific lock out/tag out procedures were not utilized by employees who were required to perform servicing tasks on ice cream manufacturing equipment. These conditions exposed employees to the hazard of being caught in the machine.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about March 18, 2015, in the production area of the Yarnell's ice cream manufacturing facility, located at 320 S Spring St in Searcy, Arkansas, a periodic inspection of the lock out/tag out program had not been conducted since the plant reopened in April 2012. As a result, employees attempted to clear a clogged fill nozzle without locking the machine out. These conditions exposed the employees to the hazard of being caught in the machine.

These conditions exposed employees to the hazards associated with coming in contact with bodily fluids.
